TO AVOID ERROR AND DELAY, READ THE FOLLOWING INSTRUCTIONS WHEN PREPARING YOUR APPLICATION.


Read the entire application before making any entries.


Have answers typewritten.


Give specific answers to each part of all questions. Do not leave any spaces blank. If the question is not applicable to you, simply indicate the same in the space provided for the answer. Please note that questions 26 and 27 of the application are applicable to all applicants. Different information is required for those applicants who have been in practice and those who have not.

Give complete dates, i.e., month, day and year, wherever possible. The Board understands that you might not remember the particulars of every job you have ever had, particularly those you had in high school or for summer employment. However, the Board does expect you to do the best you can in reconstructing your entire employment history. Those applicants who have been self-employed must include the name of two persons who can verify the existence of your business or practice, e.g., landlord, accountant. If there are periods of time when you were not employed, please identify those times those periods as well. GIVE COMPLETE ADDRESSES FOR YOUR EMPLOYERS, INCLUDING THE STREET NAME AND NUMBER, CITY AND STATE, AND ZIP CODES.

Where references having a particular relationship to you are requested or given, clearly identify the relationship, e.g. client, employer, associate, partner, professor, etc. wherever their names are used. GIVE COMPLETE ADDRESSES FOR YOUR REFERENCES, INCLUDING THE STREET NAME AND NUMBER, CITY AND STATE, AND ZIP CODES. Do not use relatives as references either for written recommendations or in the body of the application. Please note that question 26 asks for 3 references for each locality in which you have practiced or lived.

When using additional attachments to supplement answers, give the information in the same manner as is called for in the application. Reference attachments in the answer space.

NB: Incomplete applications will be returned to the applicant.
